Earth servers use a real-world map (usually 1:500 or 1:1000 scale) instead of a generated overworld. You pick a country, claim land, found towns, declare wars, and play out alternate history with thousands of players.
Most Earth servers run Towny or Dynmap-style claiming plus economy, taxes and proximity chat. They tend to be long-running - some maps have been live for 5+ years - so picking an established server means joining real history.

Players form towns using Towny, then ally towns into nations that correspond to real or fictional countries. Nations can declare war, sign treaties, and control territory. Political dynamics between player nations drive most of the server's long-term storylines.
